A parent or legal guardian completes this stage-one application for a Southern California student in grades 4 through 8. It collects only the information needed to review initial eligibility and contact the family.

Applications are open year-round. Applications received by January 31 are reviewed for decisions announced by May 1. Applications received by July 31 are reviewed for decisions announced by November 1. Applications received after a deadline move to the next review cycle.

I am at least 18 years old and I am the student's parent or legal guardian. I confirm that the student is in grades 4 through 8, lives in Southern California, and maintains a GPA of 3.5 or higher or strong equivalent grades. I consent to SELF Principle using this information to assess initial scholarship eligibility and contact me about next steps.

Securing the form...

This stage-one application must be submitted by a parent or legal guardian. It is sent to authorized SELF Principle recipients by email and is not saved as a form entry in the website database. After a successful submission, this page will show a confirmation. SELF Principle will contact the parent or legal guardian if the student is invited to continue.
