A parent or legal guardian completes this stage-one application for a Southern California student in grades 4 through 8. It collects only the information needed to review initial eligibility and contact the family.
Applications are open year-round. Applications received by January 31 are reviewed for decisions announced by May 1. Applications received by July 31 are reviewed for decisions announced by November 1. Applications received after a deadline move to the next review cycle.
Before you begin
- Parent or legal guardian name and email
- The student’s first name, grade, city, and sport
- The requested support and estimated amount, up to $500
- The league, program, or vendor, if known
- One brief description of the barrier this support would address
Do not upload or include report cards, income records, tax documents, bank information, medical information, Social Security numbers, immigration information, or other sensitive records. If the student is invited to continue, SELF Principle will provide separate secure instructions.
Stage-one application
This stage-one application must be submitted by a parent or legal guardian. It is sent to authorized SELF Principle recipients by email and is not saved as a form entry in the website database. After a successful submission, this page will show a confirmation. SELF Principle will contact the parent or legal guardian if the student is invited to continue.
